【Abstract】 Objective: To investigate the clinical effect of radical resection for locally metastatic bladder cancer after neoadjuvant chemotherapy. Methods: A retrospective analysis of 37 cases of local metastasis of bladder cancer patients which divided into observation group(20 cases) and control cases(17 cases) according to their therapy. Patients in observation group were treated by cisplatin and gemcitabine neoadjuvant chemotherapy followed by radical resection surgery and patients in control group were treated by radical resection surgery only. Observe the effect of chemotherapy, tumor size before and after, adverse reactions and survival rate. Results: The effective rate of chemotherapy was 75.0%(15/20), and the diameter of tumor was significantly decreased after treatment(P<0.01). The overall incidence of adverse reactions was 70.0%(14/20), and most of them were I or II grade. The median survival time of the observation group was 41 months which significantly higher than 26 months in control group (P<0.05). Conclusion: Neoadjuvant chemotherapy can achieve a high response rate, significantly reduce the tumor, combined with surgical treatment can prolong the survival time of patients.
