[Abstract] Objective: To explore the clinical efficacy and safety of laparoscopic radical cystectomy with orthotopic ileal neobladderin patients with bladder cancer.Methods: Selected 70 cases underwent radical cystectomy with orthotopic ileal neobladder in our hospital from 2009 January to 2014 March as the research object,including 30 cases of laparoscopic operation as the observation group,40 patients underwent open operation as control group.Compared peri operation period,the new bladder function and occurrence conditions of two groups.Results: Observation group hospitalization time,gastrointestinal function recovery time amount of bleeding was significantly reduced, compared with the control group, the difference was statistically significant (P<0.05),the operation time was significantly longer than that of the control group (P<0.05); two patients had no statistical significance in the control rate, urinary bladder internal pressure, bladder capacity, residual urine volume differences (P>0.05), maximal urinary flow rate in observation group was significantly higher than the control group (P<0.05).The main postoperative complications of two groups was intestinal obstruction, urinary fistula, urinary tract infection, pulmonary infection, dysuria, incision infection; observation group total complication rate was 16.7%, significantly lower than the control group (37.5%), a statistically significant difference between the two groups (χ2=4.642,P<0.05).Conclusion: Laparoscopic radical cystectomy with orthotopic ileal neobladder with minima1 invasion, less bleeding, rapid recovery and good bladder function,incidence rate is low, and worth of clinical promotion.
