Abstract Objective: To evaluate the therapeutic effect and safety of transurethral lithotrite, pneumatic lithotripsy and Ho:YAG laser lithotripsy in treating Benign prostatic hyperplasia (BPH) complicated with bladder stones. Method: We divided a total of 165 cases of BPH complicated with bladder stones into 3 group. In groupⅠ, 68 case were performed of transurethral lithotrite lithotripsy combined with TURP; in group Ⅱ, 51 cases received pneumatic lithotripsy combined with TUVP; in group Ⅲ, 46 cases were performed of Ho:YAG laser lithotripsy combined with bipolar plasmakinetic prostatectomy, we compared the complication and effect of lithotripsy in and, after operation. Result: In group Ⅰ, prostate and bladder neck injury happened in 19 cases, failure of lithotripsy are 23 case, another 9 cases had urethral stricture post-operation, the achievement ratio is 62.0%(42/68); As in group Ⅱ, prostate and bladder neck injury happened in 5 cases, failure of lithotripsy are 7 case, another 3 cases had urethral stricture post-operation, the achievement ratio is 80.0%(41/51); in group Ⅲ, there is no complication in the period of operation, all 46 cases succeeded in smashing the stones, only one case present urethral orifice stricture. There were remarkable differences in lithotripsy success rate and the rate of changing to open operations due to stones not crushed among three groups(P<0.05). Conclusions: Ho:YAG laser lithotripsy combined with bipolar plasmakinetic prostatectomy is a safe and effective treatment modality for bladder stones with BPH.
