Clinical efficacy between the surgery of transurethral plasmakinetic resection of the prostate and retropubic laparoscopic simple prostatectomy with prostatic urethra preservation
Objective To investigate the feasibility and clinical value of the developed surgery of retropubic laparoscopic simple prostatectomy with prostatic urethra preservation. Methods Clinical data of 104 patients who suffered from benign prostatic hyperplasia (BPH) with a large volume of prostate (> 80 mL) in the hospital from January 2013 to December 2016 were collected. And a total of 104 patients were enrolled according to the criteria for inclusion and exclusion. All patients were divided into three groups. Group A: 22 patients underwent surgery of retropubic laparoscopic simple prostatectomy with prostatic urethra preservation from January 2013 to December 2014. Group B: 57 patients underwent surgery of transurethral plasmakinetic resection of the prostate(TPKRP) from January 2013 to December 2016. Group C: 25 patients underwent surgery of retropubic laparoscopic simple prostatectomy with prostatic urethra preservation from January 2015 to December 2014. Age, prostate volume, International Prostate Symptom Score (IPSS), residual urine (RUV), maximum urinary flow rate (Qmax), the operation time and intraoperative bleeding volume were recorded and analyzed among three groups. And the time of bladder irrigation, the time of indwelling catheter, the length of stay were recorded. And the IPSS score, RUV and Qmax were analyzed 3 months after operation. Results ①The operation time was long and the bleeding was more in the early stage of the surgery of retropubic laparoscopic simple prostatectomy with prostatic urethra preservation, after the surgery was mature, the operation time was shortened obviously, and the amount of bleeding during operation was reduced. ②After the surgery was mature, the effect was similar to the surgery of TPKRP without prolonging the operation time. ③And the matured surgery could shorten the time of bladder irrigation, the days of extubation and the length of postoperative hospital stay compared with the surgery of TPKRP. Conclusions It is a safe and effective method for the treatment of large volume prostate (>80 mL) after the surgery of retropubic laparoscopic simple prostatectomy with prostatic urethra preservation is matured.
